Inside the new ‘no-filler filler’ trend

Profhilo Structura is one of the most in-demand injectable treatments right now and is set to become one of 2026’s biggest aesthetic trends. Building on the success of the original Profhilo – the revolutionary skin-booster – Profhilo Structura is designed specifically to restore volume, strengthen skin architecture, and rejuvenate areas impacted by ageing, weight loss, or skin laxity. To understand why so many women are turning to this ‘no-filler filler’, I decided to review Profhilo Structura myself and visited London’s most sought-after aesthetic specialist, Dr Brendan Khong at his clinic in London, to get his expertise and a full breakdown of the injectable treatment everyone is talking about.

What Is Profhilo Structura?

Profhilo Structura is an advanced bio-remodelling injectable that targets the superficial fat pads of the face – the structural foundations that give the face lift, shape and youthful contour. Over time and as we age, these fat pads naturally shift and deplete, leading to sagging, flattening of the mid-face, hollowing and reduced definition.

Rather than adding artificial volume like traditional dermal fillers, Profhilo Structura works by supporting and strengthening the underlying facial architecture. Dr Brendan Khong explains that “Profhilo Structura doesn’t sit in one place to create volume. It works with the natural structures of the face improving support so the tissues sit better over time.”

Profhilo vs Profhilo Structura: What’s the Difference?

Although similar, Profhilo and Profhilo Structura serve entirely different purposes in aesthetics. Classic Profhilo is best described as a powerful skin-booster. Its blend of high and low molecular weight hyaluronic acid spreads evenly through the dermis, delivering intense hydration while stimulating collagen and elastin. The result is healthier, brighter skin that looks refreshed and youthful. It does not add volume and it doesn’t reshape the face but instead, it improves skin texture, softness and overall radiance. Most practitioners recommend two treatments, spaced four weeks apart, to achieve visible results.

Dr Brendan Khong explained to me that classic Profhilo is very much a skin-quality treatment. Its hybrid formulation of high and low molecular weight hyaluronic acid allows the product to diffuse evenly through the skin rather than staying localised. This slow diffusion hydrates the upper layers of the skin while stimulating collagen and elastin production deeper within, improving elasticity, firmness and overall radiance. “Profhilo improves how the skin behaves,” Dr Brendan says. “It enhances hydration and glow but doesn’t add shape or structure.”

Profhilo Structura, however, works in a completely different way. Designed specifically for the superficial fat pads – the deeper structures that give the face its natural shape and contour – Structura helps to restore support where age-related descent begins. As we get older, these fat pads deflate and shift, causing hollowing, sagging and a general loss of facial definition. Structura addresses this by strengthening the underlying architecture, subtly lifting and repositioning the tissues without altering the natural proportions of the face.

Unlike classic Profhilo, Structura works deeper beneath the skin, targeting the fat pads that support facial contours and shape. As these structures weaken with age, the face can start to look heavier or flatter and Structura is designed to reinforce that support.

Dr Brendan says: “Profhilo Structura is designed for the facial fat pads to address age-related volume loss, helping to reposition and revitalise the superficial fat pads beneath the skin. It’s chosen for those with sagging cheeks or jawlines, a hollowed mid-face and for anyone who wants improved contour without the use of filler.”

Profhilo is about skin quality while Structura is about structural integrity. As Dr Brendan explains, “Classic Profhilo treats the skin, Profhilo Structura treats the foundation beneath it.” Both treatments typically follow the same two-session protocol, spaced four weeks apart, but their outcomes are distinctly different – one delivers hydration and glow, the other restores support and contour, all while maintaining a natural-looking and subtle results.

Profhilo Structura Review

During my Profhilo Structura consultation, we focused on the areas that had been bothering me most – in particular, the lines that run from under my eyes into my cheeks. While these are often described as under-eye bags, Dr Brendan explained that in my case they are actually malar bags.

Malar bags sit higher on the cheek and are linked to changes in the superficial fat pads. As these fat pads drop and lose support over time, they can create a shadow or fold beneath the eyes that looks heavier or more pronounced, even when there isn’t true puffiness or hollowing. Dr Brendan also explained that fluid retention can make malar bags appear worse, which is why they can look more noticeable first thing in the morning, even though the underlying issue is structural rather than fluid-based.

Dr Brendan told me “Profhilo Structura helps to replenish and rejuvenate the facial fat pads without the use of any filler” so rather than adding filler, which can easily create heaviness in this area and potentially make malar bags worse, Profhilo Structura was recommended as a way to support the natural structures of my face and soften the appearance of my malar bags in a natural-looking way.

Profhilo Structura is injected into carefully selected areas that correspond with the facial fat compartments, particularly across the mid-face and cheeks. Using a cannula, the product is placed via the preauricular cheek area and along the zygomatic arch (the cheekbone), allowing support to be delivered across the face from a single insertion point. The amount of product used can be adjusted depending on individual facial morphology and in my case Dr Brendan tailored the treatment accordingly.

We chatted throughout the treatment, with constant distraction, gentle tapping and a stress ball to squeeze, which made a real difference. Dr Brendan’s technique meant I felt very little, and the whole treatment was over far more quickly than I expected. Looking up at Dr Brendan, you really do trust that you’re in good hands – he has clearly perfected his own ‘glass skin’ approach, focused on flawless, natural-looking results.

Having had traditional Profhilo before, which I found quite uncomfortable in parts, I was genuinely surprised by how manageable Profhilo Structura felt. While there were a few moments that were slightly stingy in certain areas, it was never painful and the sensation passed quickly. Overall, the experience felt controlled, considered and far more comfortable than I’d anticipated, highlighting just how much practitioner skill, needle choice and technique matter to the overall experience.

Immediately after treatment, my face looked noticeably fuller and more lifted, particularly through the cheeks. I actually remember saying to Dr Brendan that it looked like I’d had a mini face lift. There was an instant sense of structure and lift with fullness back in my face and my skin looked fresher, subtly contoured and glowy.

Over the following days and weeks, that initial fullness settled. While I understood this was part of the process, I did find it slightly disappointing as I’d grown to like the fuller cheeks. That said, Dr Brendan had managed my expectations and explained that Profhilo Structura isn’t designed to deliver instant volume. Instead, it works gradually, supporting the natural structures of the face over time. When I returned for the second session of the recommended course, he also explained that there is always the option to add a small amount of filler later if I want that initial fullness back but that it’s better to start by strengthening the foundation first and build from there.

Based on my experience and Dr Brendan Khong’s guidance, Profhilo Structura is particularly well suited to women experiencing early to moderate mid-face sagging, those with malar bags linked to fat-pad changes, and people who have noticed facial flattening following weight loss. It also works well for anyone seeking subtle lift and support without committing to traditional filler. As Dr Brendan notes, because Profhilo Structura is not a dermal filler, results are not immediate and for most patients, visible improvement develops gradually and typically becomes noticeable around four to eight weeks after the second treatment.

Like my own experience, many report visible fullness immediately after treatment, followed by a settling phase and then gradual improvements in firmness, lift and facial balance over the following weeks, particularly after the second session.
